AS THE ADOPTING PARTY, I AGREE TO THE FOLLOWING PROVISIONS:

1.It is our policy that adult cats and kittens must be spayed or neutered and given appropriate vaccinations before adoption and home delivery. We require home delivery on cats and kittens adopted through our organization and a home inspection before they are placed. No cat or kitten will be relinquished the same day the adoption agreement is signed.

2. I agree to keep an identification tag attached to a properly fitted collar, which will remain on the cat
at all times and arrange for microchipping.

3.I agree to provide the adopted pet with necessary inoculation at the intervals advised by my veterinarian and routine physical examinations.

4. I agree to provide nourishing food to the adopted pet and understand the basics of nutrition.

5.Since the adopted pet is a cat and/or kitten, I agree to keep it INDOORS, which means it will never be allowed outside my dwelling. I also understand that under no circumstances will it be declawed.

6.If for any reason I cannot keep the adopted pet, I agree to notify FCCC IMMEDIATELY and return the pet upon request. There is no time limit on returning a pet to FCCC.

7.I agree not to abuse or neglect the adopted pet and I authorize FCCC, at their discretion, to determine whether or not the pet has been abused or neglected.

8.I understand that any failure to perform the provisions under the foregoing agreement will constitute a breach of contract. In the event of such breach of contract, I authorize FCCC to reclaim both possession and ownership of the Adopted pet.

9.I understand that the pet covered by these adoption papers is, as far as can be determined by FCCC,
is in good health, has had a physical examination and any additional treatment necessary. FCCC will provide the medical history of the pet to the adopter. FCCC is not responsible for any medical fees incurred after adoption date. However, if a health problem develops, I will notify FCCC to discuss the matter.

10.I agree to give FCCC visitation rights to ensure the terms of this adoption agreement are being observed.

11.Compassion note: If the kitten being adopted was born of a feral Mother it may require special handling and care, beyond that which is usually needed for domestic kittens born in a secure enviornment to a domestic Mother. See Domesticating Kittens www.feralcatcaretakers.org

13. If there are any problems or advice is required in caring for the adopted pet, I will feel free to contact
FCCC.

14. I AGREE THAT UNDER NO CIRCUMSTANCES WILL THE CAT OR KITTENS BE RE-ADOPTED TO A SECOND PARTY OR TAKEN TO A CITY OR PRIVATE SHELTER. IF, FOR SOME REASON IT IS DECIDED BY THE ADOPTER THAT THE CAT OR KITTEN CAN NO LONGER BE KEPT IN THE PRESENT CIRCUMSTANCES, FCCC MUST BE CONTACTED IMMEDIATELY AND THE CAT/S OR KITTEN/S MUST BE GIVEN UP TO THEM FOR CONTINUED CARE. VIOLATION OF THIS AGREEMENT WILL NECESSITATE LEGAL ACTION UPON THE ADOPTER.
